Job summary

CenterWell Home Health is seeking a licensed Occupational Therapist to provide in-home therapy in Pullman, WA. You will screen and treat patients, develop rehab plans, and collaborate with physicians and the care team to support functional recovery and independence.

This role emphasizes home-based rehabilitation, documentation, and care coordination to optimize outcomes. The position requires OT licensure, CPR, and a valid driver’s license with reliable transportation; benefits start day one,

Qualifications

Current and unrestricted OT licensure.
Current CPR certification.
Valid driver’s license with reliable transportation.

Responsibilities

Assess and screen patients’ daily living and rehab needs with measurable objectives.
Collaborate with physicians and the care team on the Plan of Care.
Provide therapy to improve strength, range of motion, coordination and function at home.
Document progress and update care plans; participate in discharge planning.
Design and supervise retraining programs and train caregivers.
Coordinate with vocational team for reintegration into home/work environments.
Maintain accurate patient records and ensure physician orders are implemented.
