Job summary

CenterWell Home Health seeks a Home Health Occupational Therapist to assess and retrain patients in their homes, coordinating with physicians and the care team to restore function and independence.

Responsibilities include creating Plan of Care updates, delivering therapy per physician orders, and instructing patients and families in home-based rehabilitation. Valid OT licensure, CPR, and reliable transportation required.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with the patient's physician and other health care team members and participate in development/revision of the Plan of Care Treatment.
• Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve/restore strength, coordination, range-of-motion and function or teach compensation measures.
• Review/expand retraining programs in line with behavioral goals and rehabilitation program guidelines.
• Consult with vocational team members to develop vocational plans for patients.
• Report patient status and progress to physicians and care teams.
