Job Summary

Our agency is expanding, and we are looking for an experienced Occupational Therapist to provide home health services focused on improving patient independence and safety within their home environment. Responsibilities

• Conduct in-home evaluations and functional assessments
• Develop and implement individualized treatment plans
• Assist patients with ADLs (bathing, dressing, grooming, toileting)
• Train patients on adaptive equipment and home safety modifications
• Complete OASIS documentation as required
• Collaborate with nurses, PTs, and other team members

Qualifications

• Graduate of an accredited Occupational Therapy program
• Active Florida OT license
• Home health experience preferred
• OASIS experience strongly preferred
• Strong documentation and time management skills
• Valid driver’s license and reliable transportation
