Job summary

Palmshomecare is seeking an experienced Occupational Therapist to provide in-home health services, focused on improving patient independence and safety within their home environment.

You will evaluate patients in their homes, develop and implement individualized treatment plans, and collaborate with nurses and other team members to enhance functional outcomes.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Active Florida OT license.
• Home health experience preferred.
• OASIS experience strongly preferred.
• Strong documentation and time management skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Train patients on adaptive equipment and home safety modifications
