Job summary

Palms Home Care in Florida is seeking an experienced Occupational Therapist to provide in-home health services focused on improving patient independence and safety within the home environment. You will conduct in-home evaluations, develop treatment plans, assist with ADLs, train on adaptive equipment and home safety, and complete OASIS documentation while coordinating with the care team.

Flexible scheduling and mileage reimbursement are provided; a Level 2 background screening is required for

Qualifications

• Graduate of an accredited Occupational Therapy program
• Active Florida OT license
• Home health experience preferred
• OASIS experience strongly preferred
• Strong documentation and time management skills
• Valid driver's license and reliable transportation

• Assist patients with ADLs (bathing, dressing, grooming, toileting)
• Train patients on adaptive equipment and home safety modifications
