Job summary

Health Wealth Safe is seeking an Occupational Therapist to assess, teach, and deliver therapy interventions in a home health setting, collaborating with physicians within an interdisciplinary team.

You will conduct initial and ongoing assessments, develop written plans of care, and document outcomes while educating patients and caregivers on home exercises and discharge plans.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Current, unrestricted state licensure as an OT.
• Valid CPR, Driver’s License, and proof of auto insurance.

Responsibilities

• Conducting initial and ongoing assessments of mobility, coordination, strength, and function.
• Delivering occupational therapy services in accordance with physician orders.
• Maintaining documentation, planning discharges, and updating plans of care.
• Educating patients and caregivers on home exercise and discharge plans.
• Coordinating services with healthcare team members, physicians, and families.
• Supervising OT Assistants or Home Health Aides as applicable.
• Completing OASIS assessments where permitted by regulations.

Essential Duties Include

• Conducting initial and ongoing assessments of mobility, coordination, strength, and function.
• Administering and interpreting tests to set functional goals and outcomes.
• Creating written evaluations and plans of care outlining therapy needs, goals, progress, and expected results.
• Delivering occupational therapy services in accordance with physician orders.
• Maintaining documentation, modifying treatment plans, and planning discharges.
• Educating patients and caregivers on home exercise and discharge plans.
• Coordinating services with healthcare team members, physicians, and families.
• Supervising other personnel such as OT Assistants or Home Health Aides as applicable.
• Completing OASIS assessments where permitted by regulations.
