Job summary

Palms-Home-Care seeks an experienced Occupational Therapist to provide in-home health services focused on improving patient independence and safety in the Florida Clearwater area. The role emphasizes comprehensive assessments and collaborative care.

You will conduct in-home evaluations, develop and implement individualized plans, assist patients with ADLs, train on adaptive equipment and home safety, and complete OASIS documentation.

Qualifications

• Graduate of an accredited Occupational Therapy program
• Active Florida OT license
• Home health experience preferred
• OASIS experience strongly preferred
• Strong documentation and time management skills
• Valid driver’s license and reliable transportation

Responsibilities
