Job summary

CenterWell Home Health is seeking an Occupational Therapist to support patients at home, focusing on rehabilitation and functional improvement. You will assess clients, develop retraining plans, coordinate with physicians and the care team, and supervise aides to ensure high-quality in-home therapy services.

A valid OT license and CPR certification are required. Responsibilities include documenting progress, participating in discharge planning, and designing activities to promote independence

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Assess daily living/work-related skills and develop retraining programs with measurable objectives.
• Confer with physician and participate in Plan of Care Treatment development/revision.
• Provide therapeutic treatment and instruction per physician orders to improve strength, coordination, range of motion and function.
• Review and expand retraining programs to align with behavioral goals and rehab program guidelines.
• Consult with vocational team members to develop and implement vocational plans.
• Report patient status and progress to physician, rehabilitation staff, patient and family.
• Participate in care coordination and discharge planning and act as a resource to health care personnel.
• Design community reintegration activities to assist the client in physical reconditioning and coordination with the interdisciplinary team.
• Recommend/adapt special equipment to improve residence or work environments and participation in rehabilitation.
• Train staff and family to carry out retraining, including dressing, feeding, grooming and hygiene skills; participate in team/family meetings.
• Supervise OT Assistant and Home Health Aide performance in delivering services.
• Accurately document care observations, interventions and evaluations; submit plans and discharge summaries to supervisor.
