CenterWell Home Health seeks a Home Health Occupational Therapist to assess, retrain and improve patients' daily living skills in home settings. You will design and monitor therapy plans, collaborate with physicians and care teams, and help patients regain independence.

Responsibilities include documenting progress, coordinating discharge planning, and providing staff training on retraining programs. A valid OT license and CPR certification are required, along with a valid driver's license and

Formación

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsabilidades

• Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs.
• Confer with the patient's physician and care team to develop/revise the Plan of Care.
• Provide therapeutic treatment to improve/restore strength, coordination, ROM and function.
• Review/revise retraining programs to meet patient goals within the rehab program guidelines.
• Document patient status, progress and care observations in day services records.
