Job summary

Amedisys, part of the Optum family, is seeking an Occupational Therapist to evaluate and treat patients in their homes, promoting independence, safety, and improved quality of life. The role collaborates with patients, families, and an interdisciplinary team to develop individualized plans per physician orders and agency policies.

Primary responsibilities include delivering OT services, documenting thoroughly, and ensuring safety.

Qualifications

• 1+ years of related occupational therapy experience to ensure knowledge and skills are sufficient
• Current CPR certification to ensure patient safety during home-based care
• Driver's License and access to reliable transportation for home visits

Responsibilities

• Responsible for all aspects of occupational therapy service delivery under a physician-approved plan of care
• Ensures the plan of care addresses all problems identified in the assessment or documents rationale if identified problems are not addressed in the plan of care
• Accountable for the safety and effectiveness of the occupational therapy service delivery process
• Completes documentation timely and accurately
• Supervises home health aide when RN is not active in the case
• Other duties as assigned
