UnitedHealth Group, through its Amedisys/Home health network, is seeking an Occupational Therapist to evaluate and treat patients in their homes to promote independence, safety and improved quality of life. You will work with patients, families, and a multidisciplinary team to create individualized treatment plans based on physician orders and agency policies.

Primary responsibilities include delivering OT services under a physician-approved plan of care, ensuring the plan addresses identified

Qualifications

• Current license to practice occupational therapy in the assigned state.
• Graduate of an ACOTE-accredited OT program.
• Eligible to take or have completed NBCOT certification.
• One year related OT experience or approval from regional clinical leaders if less than one year.
• Current CPR certification.

Responsabilités

• Responsible for all aspects of OT service delivery under a physician-approved plan of care.
• Ensures plan of care addresses all problems identified in the assessment and documents rationale if not addressed.
• Accountable for safety and effectiveness of OT service delivery.
• Completes documentation timely and accurately.
• Supervises home health aide when RN is not active in the case.
• Other duties as assigned.
