Job summary

VitalCaring Group in Florida is seeking a skilled Occupational Therapist to join our home health team. You will evaluate patients at home, design personalized treatment plans, and help people regain independence while delivering compassionate, patient-centered care.

This full-time, field-based role requires NBCOT certification, an active state OT license, CPR, and a valid driver's license. You will collaborate with a multidisciplinary team, supervise COTAs, and participate in discharge planning

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification.
• Current state OT license.
• CPR certification.
• Valid driver's license and reliable transportation.
• Minimum of 1 year home health experience preferred.

Responsibilities

• Evaluate patients' functional status and safety at home.
• Develop individualized treatment plans to improve independence.
• Provide restorative therapy and retraining in daily living activities.
• Fabricate splints and train patients in adaptive devices.
• Assist patients returning to work with functional/work capacity evaluations.
• Educate patients and families on plans and needs.
• Collaborate with interdisciplinary team to coordinate services.
• Participate in discharge planning.
