Job summary

VitalCaring Group in Austin, Texas seeks a licensed Occupational Therapist to provide high-quality home health services. You will partner with patients, families, and interdisciplinary teams to maximize independence and safety in daily activities.

Responsibilities include evaluating functional status, designing individualized treatment plans, training on daily living activities, and coordinating care through EMR documentation and discharge planning.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification and state OT licensure.
• Minimum one year home health experience preferred.
• CPR certification and valid driver’s license.

Responsibilities

• Evaluate patients’ functional status at home and set measurable goals.
• Assess home environments for safety risks and recommend modifications.
• Develop and implement individualized treatment plans to improve independence.
• Provide training in activities of daily living and use of adaptive devices.
• Collaborate with families and the care team for seamless, patient-centered care.
• Document care in EMR and participate in discharge planning.
