VitalCaring is seeking a licensed Occupational Therapist (OT) for our Home Health team in Texas. You will evaluate patients, design personalized treatment plans, and work with families to promote independence in daily living at home.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification.
• Current state licensure as an Occupational Therapist (TX).

Responsibilities

• Evaluate patients’ functional status and activities of daily living.
• Assess home environments for safety and recommend adaptations.
• Develop and implement individualized treatment plans with measurable goals.
• Provide therapeutic interventions to restore function and independence.
• Educate patients and caregivers on plans and safety.
• Coordinate with care team for seamless patient-centered care.
