Job summary

VitalCaring is seeking a Home Health Occupational Therapist (OT) for field-based, full-time roles in Temple, TX. You will evaluate patients at home, develop personalized treatment plans, and collaborate with families and the care team to restore independence and safety in daily activities.

Qualifications

• Licensed occupational therapist with NBCOT certification
• Current state OT license in good standing
• CPR certification required
• Valid driver’s license and reliable transportation

Responsibilities

• Evaluate patients’ functional status and home safety to guide therapy
• Develop individualized treatment plans with measurable goals
• Provide therapeutic interventions to restore independence in daily tasks
• Fabricate splints and train patients in adaptive equipment as appropriate
• Educate patients and families on treatment plans and safety
• Coordinate with care team and document in EMR
• Assist in discharge planning and transitions of care
• Supervise COTAs as applicable and oversee home health aide care when required
• Collaborate with physicians and interdisciplinary teams to ensure seamless care
