Job summary

Best Home Health Providers Inc. is seeking an Occupational Therapist to support patients at home across the Bay Area. You will assess functional abilities, create personalized treatment plans, and guide patients to regain independence and safely navigate daily tasks.

This per diem role offers a rate of $130 per visit, with opportunities to convert to full-time based on performance. A California OT license, NBCOT registration, CPR/AHA, and driving to patient homes are required.

Qualifications

• Graduate of an occupational therapy curriculum accredited by AMA/AOTA.
• Registered with NBCOT (National Registration Examination).
• Active California OT license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification (AHA).
• Valid driver's license and reliable insured vehicle; ability to travel to patient homes.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and promptly report changes in patient condition.
• Coordinate with the broader care team to ensure seamless, patient-centered care.
• Educate family members and caregivers on the patient’s treatment approach.
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.
