Job summary

Best Home Health Providers Inc. is seeking an Occupational Therapist to help patients regain independence in their homes across the Bay Area and surrounding regions.

This per diem role emphasizes high-quality, patient-centered care with a supportive, family-oriented team. You will evaluate functional abilities, develop personalized treatment plans, teach daily living skills, coordinate with physicians, and supervise OT assistants.

Qualifications

• OT degree from an accredited program
• NBCOT registration
• Active California OT license and CPR certification (AHA)

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend adaptive equipment and orthotic devices
• Maintain accurate clinical records and report changes in patient condition
• Coordinate with the care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the treatment approach
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition home
