Job summary

Best Home Health Providers Inc. in California seeks an Occupational Therapist to support patients at home, helping them regain independence and improve daily function. This role emphasizes patient-centered care and safe home assessments.

The position is per diem with a rate of $110-$140 per visit. Candidates must have CA OT license, active CPR, and at least 2 years of clinical OT experience; drivers license and reliable vehicle are required.

Qualifications

• Graduation from an occupational therapy curriculum accredited by relevant U.S. bodies.
• Registered with the appropriate national or state board for OT in California.
• Active CPR certification (AHA) and clean driving record.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Instruct patients on adaptive equipment and devices
• Maintain clinical records and report changes in patient condition
• Coordinate with the care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the treatment approach
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition home
• Perform other duties as required
