Job summary

Providence at Home with Compassus is seeking a Home-Based Acute Care Occupational Therapist to deliver inpatient-level therapy in patients' homes across Thurston County, WA. You will perform comprehensive assessments, craft individualized treatment plans, and coordinate discharge planning with a multidisciplinary team.

Qualifications

• Required: Bachelor's or Master's degree in Occupational Therapy.
• Preferred: 1 year of OT experience with adults in home care.

Responsibilities

• Deliver comprehensive therapy services including assessment, intervention, and discharge planning.
• Evaluate physical, cognitive, and psychosocial functions for safe home functioning.
• Develop individualized care plans with patients and families.
• Refer to medical, social, and emotional factors impacting care.
• Educate and counsel clients on treatment plans and safety within scope.
• Supervise Home Health Aides and manage visits with the care team.
• Complete same-day documentation and support onboarding of new staff.
