Job summary

Providence at Home with Compassus in Washington state is seeking a Home-Based Acute Care Occupational Therapist to bring inpatient-level therapy into patients' homes. You will evaluate, treat, and discharge plan with an emphasis on independence and safety, working independently within an interdisciplinary team.

Responsibilities include comprehensive assessments, personalized care plans, in-home therapy, caregiver education, and documenting visits the same day to meet agency guidelines.

Qualifications

• Bachelor's or Master's degree in Occupational Therapy required.
• Current State OT license required.
• 1 year home health OT experience preferred.

Responsibilities

• Deliver comprehensive in-home OT services including assessment, treatment, and discharge planning.
• Evaluate physical, cognitive, and functional abilities to support home safety.
• Develop individualized care plans with patients and families.
• Educate and coach clients and caregivers on treatment goals and safety.
• Supervise Home Health Aides and coordinate with the interdisciplinary team.
