Qualifications

Responsibilities

• Deliver comprehensive home-based OT services including assessment and discharge planning.
• Evaluate physical, cognitive, psychological, and perceptual functions for safe home functioning.
• Develop individualized care plans with patients and families.
• Supervise Home Health Aides and oversee COTAs (every 5th visit).
• Document visits the same day and support staff onboarding.
