Job summary

Compassus, in partnership with Providence, seeks a Home Health Occupational Therapist to deliver inpatient-level therapy in patients' homes around Mason County, WA. You’ll evaluate, plan, implement, and discharge care with an emphasis on restoring independence.

You’ll work independently within a multidisciplinary team, manage a caseload, supervise aides, and ensure timely documentation. This role supports the home-based care model across a growing network.

Qualifications

• Bachelor's or Master’s degree in Occupational Therapy is required.
• Current, unencumbered OT license valid in the state of practice.
• BLS certification required; CPR training.
• 1 year of OT experience with adults in home care preferred.

Responsibilities

• Deliver comprehensive OT services, including assessment, intervention, care planning, implementation, and discharge planning.
• Evaluate patients’ physical, cognitive, and functional status to ensure safe home-based functioning.
• Develop individualized plans of care with patients and families.
• Identify referrals related to medical, social, and emotional factors affecting care.
• Educate patients and families on treatment plans, safety, and goals within scope of practice.
• Adapt equipment and therapy techniques for the home environment and safety.
• Independently manage caseload, schedule visits, and coordinate home visits with the team.
• Organize weekly schedule balancing client visits and required team meetings.
• Supervise Home Health Aides on care plans and skill-building activities.
• Provide supervision and case management for clients seen by COTAs, completing every fifth visit.
• Deliver in-services on new treatments or techniques to staff.
• Complete documentation same day as visits per guidelines and reimbursement rules.
• Assist onboarding of new team members with clinical practice orientation.
