Providence at Home with Compassus is seeking a Home-Based Acute Care Occupational Therapist to deliver inpatient-level therapy in the home. You will conduct comprehensive evaluations, develop individualized care plans, and manage a caseload with independence while collaborating with a multidisciplinary team.

The role requires OT licensure, a BS/MS in Occupational Therapy, and the ability to supervise home health aides and COTAs. Compensation ranges reflect base pay with potential incentives.

Qualifications

• Required: Bachelor's degree Occupational Therapy or Master's degree Occupational Therapy.
• Preferred: 1 year of Occupational Therapy experience with adults in a home care setting.
• Current, unencumbered OT license in State of Employment; BLS within 30 days of hire.

Responsabilités

• Delivering comprehensive therapy services including assessment, intervention, care planning, implementation, and discharge planning.
• Evaluating clients' physical, cognitive, psychological, and perceptual functions to ensure safe functioning at home.
• Developing individualized plans of care in collaboration with patients and families.
• Identifying and referring related medical, social, and emotional factors affecting care.
• Educating and counseling clients and families on treatment plans, safety concerns, and goals.
• Supervising Home Health Aides on care plans and training; providing supervision for COTAs (every fifth visit).
• Completing documentation on the same day as visits per agency guidelines.
