Job summary

Providence at Home with Compassus is seeking a Home-Based Acute Care Occupational Therapist to deliver inpatient-level OT services directly in patients' homes. You will perform comprehensive evaluations, create individualized treatment plans, and guide safe discharge planning while working with a multidisciplinary team.

You will manage a caseload independently, coordinate home visits, supervise aides, and participate in team meetings to ensure cohesive, high-quality care for adults transitioning

Qualifications

• Licensed OT in the United States.
• National Provider BLS - American Heart Association certification within 30 days of hire.
• 1 year of OT experience with adults in home care preferred.

Responsibilities

• Deliver comprehensive OT services including assessment, plan of care, intervention, and discharge planning.
• Evaluate clients’ physical, cognitive, and functional abilities for safe home functioning.
• Develop individualized care plans with patients and families.
• Refer to medical, social, or emotional factors impacting care.
• Educate patients and families on treatment plans and safety within scope of practice.
