Job summary

Providence at Home with Compassus seeks a Home-Based Acute Care Occupational Therapist to bring inpatient-level therapy into patients' homes in the Shelton, WA area. You will evaluate, treat, and discharge plan with a multidisciplinary team, ensuring continuity of care and patient independence.

Responsibilities include home visits, documentation the same day, and educating families on safety and therapy goals while supervising aides and coordinating with COTAs.

Qualifications

• Bachelor's degree or Master's degree in Occupational Therapy.
• Current, unencumbered OT license in the state of employment.
• BLS certification within 30 days of hire (American Heart Association).
• Preferred: 1 year of OT experience with adults in home care.

Responsibilities

• Provide comprehensive OT evaluations and individualized treatment plans in the home.
• Deliver inpatient-level therapy in a home setting.
• Discharge planning and collaboration with multi-disciplinary team.
• Educate patients and families on safety, home adaptations, and therapy goals.
• Supervise Home Health Aides and coordinate with COTAs as needed.
• Complete same-day documentation per agency guidelines.
