Job summary
Your Health Organization in South Carolina seeks a Nurse Practitioner to deliver primary care and acute visits across homes, ALFs, and ILFs within assigned service areas. The NP will assess, diagnose, and treat patients in collaboration with physicians, developing and implementing treatment plans and coordinating referrals as needed.
The position is salary-based, full-time with shifts including 12-hour and 8-hour options, and offers a monthly bonus.
Qualifications
- Master's or Doctorate degree in Nursing from an accredited program.
- Current SC NP licensure.
- Certification as NP in a specialty area (e.g., Family, Adult‑Gerontology, Pediatrics).
- 2 years of NP experience preferred but not required.
- Strong clinical assessment and diagnostic skills.
- Proficiency in EMR systems and healthcare tech.
- Commitment to professional development.
- CPR certification and related certifications as required.
Responsibilities
- Provide advanced nursing care within scope of practice.
- Assess, diagnose, and treat patients in various settings (homes, clinics, etc.).
- Develop and implement treatment plans including medications and therapies.
- Educate patients and families on health conditions and preventive care.
- Coordinate referrals and collaborate with healthcare team.
- Maintain accurate patient records and documentation.
