Job summary

The Medical Coder reviews patient records and provider documentation to assign ICD-10-CM, CPT, and HCPCS codes for billing, reimbursement, reporting, and regulatory compliance. Accurate coding supports clean claims and overall revenue integrity.

Preferred candidates have home health or hospice coding experience; CPC or CCS certification is a plus. You will collaborate with clinical and billing teams, stay updated on coding changes, and help improve revenue cycle performance.

Qualifications

• High school diploma or equivalent required; associate degree in Health Information Management or related field preferred.
• CPC, CCS, or equivalent coding certification preferred.
• 1–3 years of medical coding experience preferred.
• Knowledge of medical terminology, anatomy, physiology, and reimbursement methodologies.
• Experience with EMR/EHR systems and billing software.

Responsibilities

• Review patient medical records and provider documentation for completeness and accuracy.
• Assign diagnosis and procedures codes using ICD-10-CM, CPT, and HCPCS.
• Ensure coding accuracy and compliance with payer, federal, and organizational guidelines.
• Identify and resolve coding edits, denials, and documentation discrepancies.
• Collaborate with clinical and administrative staff to clarify documentation.
