Job summary

Elara Caring is hiring a Licensed Practical Nurse to provide skilled home care in the Palmer and Monson area. You will work with an RN Case Manager and clinical team to deliver care in patients' homes Monday through Friday, 8 a.m. to 5 p.m.

You'll assess patients, teach families, document visits, and coordinate with the care team to prevent hospital visits. A Massachusetts LPN license and at least one year of nursing experience are required.

Qualifications

• Massachusetts LPN license required.
• At least 1 year of clinical nursing experience.
• Valid driver's license and reliable transportation.
• Ability to travel throughout Palmer and Monson territory.

Responsibilities

• Provide nursing care in patients' homes according to the plan of care.
• Monitor patients and report changes to the RN Case Manager.
• Educate patients and caregivers on at-home care.
• Document visits, interventions, and medication updates.
• Collaborate with the care team to help patients avoid unnecessary hospital visits.

At Elara Caring, we're looking for a Licensed Practical Nurse who wants to bring skilled, personal care to patients in the Palmer and Monson area. You'll work alongside an RN Case Manager and a supportive clinical team while getting to know the people you care for.

What Your Day Could Include

• Providing nursing care in patients' homes according to their plan of care
• Monitoring patients and sharing changes with the RN Case Manager
• Teaching patients and caregivers how to manage care at home
• Documenting visits, interventions, and medication updates
• Working with the care team to help patients avoid unnecessary hospital visits
