Job summary

Elara Caring is hiring an LPN/LVN for our Indiana home health team. You will follow patient care plans, monitor progress, and educate families on self-care, while working closely with the RN Case Manager to adjust plans as patient needs change.

Ideal candidates have an active Indiana license, a nursing degree and at least 1 year of clinical care experience. Travel to patient homes is required with a focus on safe, compassionate care.

Qualifications

• Active Indiana LPN/LVN license
• Graduate of accredited nursing program (Certificate/Diploma/Associate)
• 1 year of clinical care experience
• Physical ability to lift up to 50 pounds and assist patient transfers

Responsibilities

• Follow each patient's care plan and physician orders.

Job Description:

• Follow each patient's individualized care plan and physician orders.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
