Job summary

Elara Caring is seeking a licensed practical nurse (LPN) to provide in-home patient care across assigned visits. You will follow individualized care plans, administer medications, and coordinate with the care team to ensure safety and positive outcomes.

Qualifications

• Current State License as a Licensed Vocational/Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting.
• Must possess a valid driver's license or reliable transportation to travel between patient homes.

Responsibilities

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
