Job summary

Elara Caring is seeking a Licensed Vocational Nurse (LVN) for PRN home health in Holyoke area, MA. You will follow individualized care plans, monitor patient progress, educate patients and families, and collaborate with the RN Case Manager to adjust plans as needs evolve.

Required: active LVN license in MA, 1 year of clinical care experience, and ability to lift up to 50 pounds. Driver's license or reliable transportation is needed; hospice/home health experience preferred.

Qualifications

• Current State License as LVN/LPN is required.
• Graduate of an accredited nursing program (Certificate/Diploma/Associate).
• Minimum 1 year of clinical care experience.
• Ability to lift/push/pull up to 50 pounds and assist with patient transfers.

Responsibilities

• Follow each patient's care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Educate patients and caregivers on self-care techniques.
• Collaborate with the RN Case Manager to update plans as needs change.
• Document medication regimens and update medication profiles.

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• "Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations."
• Experience in a hospice or home health environment is preferred

The compensation range is $30 Per Point, which can include additional earnings for work beyond quota, bonuses, on-call, or other performance-based compensation variables. Elara Caring determines compensation based on education, job related knowledge, skills, training, and experience.
