Job summary

Elara Caring is seeking a dedicated Licensed Practical Nurse to join our home health team in Massachusetts. You will provide nursing care in patients' homes within the Palmer and Monson area, following each patient’s plan of care and coordinating with an RN Case Manager.

The position is full-time, Monday–Friday, 8 a.m.–5 p.m., with compensation of $30–$38 per hour, based on experience, plus a robust benefits package and opportunities for career growth.

Qualifications

• Massachusetts LPN license is required.
• Graduation from an accredited nursing program is required.
• Minimum of 1 year of clinical nursing experience is preferred.
• Excellent communication and teamwork skills are essential.

Responsibilities

• Provide nursing care in patients' homes according to each patient's plan of care.
• Monitor patients and share changes with the RN Case Manager.
• Teach patients and caregivers how to manage care at home.
• Document visits, interventions, and medication updates.
• Collaborate with the care team to help patients avoid unnecessary hospital visits.
