Job summary

Elara Caring is seeking a Licensed Practical Nurse (LPN) for full-time home health visits in Michigan. You will follow individualized care plans, monitor patient progress, and coordinate with the RN Case Manager to update plans as needs change.

The role requires an active LPN license, at least one year of clinical experience, ability to lift up to 50 pounds, and reliable transportation to travel between patient homes. Hospice or home health experience is preferred.

• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
