Job summary

Elara Caring is seeking a Licensed Practical Nurse (LPN) to deliver skilled home health care in patients' homes across Massachusetts. This full-time role runs Monday–Friday, 8 a.m.–5 p.m., with no weekends or on-call; compensation is $38-$42 per hour based on experience.

You will work closely with an RN Case Manager and a supportive clinical team, performing visits, following care plans, and teaching patients and caregivers to support independence.

Qualifications

• Active Massachusetts LPN license.
• Graduation from an accredited nursing program.
• At least one year of clinical nursing experience.

Responsibilities

• Provide skilled nursing care according to patient care plans and orders.
• Monitor patient progress and report changes to the RN Case Manager.
• Teach patients and caregivers techniques to support health and independence at home.
• Collaborate with the RN Case Manager as care plans change.
• Document visits, interventions, and medication information accurately.
• Communicate with physicians, patients, families and the clinical team to coordinate care.

What You'll Do

• Provide skilled nursing care according to each patient's individualized care plan and physician orders.
• Monitor patient progress and promptly report changes in condition or safety concerns to the RN Case Manager.
• Teach patients and caregivers techniques that support health, safety, and independence at home.
• Collaborate with the RN Case Manager as patient needs and care plans change.
• Document visits, nursing interventions, and medication information accurately and promptly.
• Communicate with physicians, patients, families, and the clinical team to keep care coordinated.
• Help reduce avoidable hospitalizations through attentive care and early recognition of concerns.

What You'll Bring

• An active Massachusetts LPN license
• Graduation from an accredited nursing program
• At least one year of clinical nursing experience
• A valid driver's license, reliable transportation, and current auto insurance
• Ability to travel between patient homes and assigned work locations
• Ability to sit, stand, bend, lift, and move intermittently, including lifting 50-100 lbs.
