Job summary

Elara Caring is seeking an LPN to provide compassionate home health care in the Boston territory. Enjoy a steady Monday–Friday schedule from 8 a.m. to 5 p.m. and hourly pay between $38 and $42, depending on experience.

You’ll work closely with an RN Case Manager and clinical team, delivering one-on-one visits, documenting care, and supporting patient independence in patients’ homes. Benefit from medical/dental/vision coverage, PTO, holidays, and 401(k) with matching.

Qualifications

• Active Massachusetts LPN license.
• Graduation from an accredited nursing program.
• Minimum one year of clinical nursing experience.
• Valid driver’s license and reliable transportation.
• Ability to travel throughout the Boston territory.

Responsibilities

• Provide skilled nursing care per care plan and physician orders.
• Monitor patient progress and report changes to the RN Case Manager.
• Teach patients and caregivers self-care techniques that support health, safety, and independence.
• Collaborate with the RN Case Manager as patient needs and care plans change.
• Document visits, nursing interventions, and medication information accurately and promptly.
• Communicate with physicians, families, and the clinical team to keep care coordinated.
• Help reduce avoidable hospitalizations through attentive care and early recognition of concerns.

What You’ll Do

• Document visits, nursing interventions, and medication information accurately and promptly.

What You’ll Bring

• An active Massachusetts LPN license
• Graduation from an accredited nursing program
• At least one year of clinical nursing experience
• A valid driver’s license, reliable transportation, and current auto insurance
• Ability to travel throughout the Boston territory
• Ability to sit, stand, bend, lift, and move intermittently, including lifting 50–100 lbs.
