Job summary

Elara Caring in Massachusetts is seeking a Home Health Licensed Practical Nurse (LPN) for a full-time, Monday–Friday schedule from 8 a.m. to 5 p.m.

You will provide one-on-one skilled nursing visits in patients' homes, collaborating with an RN Case Manager to deliver compassionate care and monitor progress. Requirements include an active MA LPN license, graduation from an accredited nursing program, and at least one year of clinical nursing experience.

Qualifications

• Active Massachusetts LPN license.
• Graduation from an accredited nursing program.
• At least one year of clinical nursing experience.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide skilled nursing care per individualized care plans and orders.
• Monitor progress and report changes to the RN Case Manager.
• Teach patients and caregivers techniques to support health and independence.
• Collaborate with the RN Case Manager as plans change.
• Document visits, interventions, and medication information accurately.
• Communicate with physicians, patients and families to coordinate care.
• Help reduce avoidable hospitalizations through attentive care.

What You'll Do

• Provide skilled nursing care according to each patient's individualized care plan and physician orders.
• Monitor patient progress and promptly report changes in condition or safety concerns to the RN Case Manager.
• Teach patients and caregivers techniques that support health, safety, and independence at home.
• Collaborate with the RN Case Manager as patient needs and care plans change.
• Document visits, nursing interventions, and medication information accurately and promptly.
• Communicate with physicians, patients, families, and the clinical team to keep care coordinated.
• Help reduce avoidable hospitalizations through attentive care and early recognition of concerns.

What You'll Bring

• An active Massachusetts LPN license
• Graduation from an accredited nursing program
• At least one year of clinical nursing experience
• A valid driver's license, reliable transportation, and current auto insurance
• Ability to travel between patient homes and assigned work locations
• Ability to sit, stand, bend, lift, and move intermittently, including lifting 50-100 lbs.
