CenterWell Home Health seeks a compassionate nurse to provide one-on-one home care, reporting to the Clinical Manager. You will manage care plans with physicians and clinicians to support recovery and help patients return to their daily lives.
Responsibilities include delivering skilled nursing care in patients’ homes, implementing care plans, educating families, and documenting care and outcomes. Strong communication with the care team ensures timely updates and coordinated discharge planning.
As a CenterWell Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you’ll manage care plans that support recovery and help patients get back to the life they love.
TB Statement: This role is considered patient‑facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Driving Statement: This role requires a valid state driver’s license and personal vehicle liability insurance. Individual must carry insurance in accordance with their residing state minimum required limits (or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher).
Scheduled Weekly Hours: 1 hour.
