Job summary

CenterWell Home Health is seeking a compassionate licensed practical nurse or licensed vocational nurse to join our patient‑centered team. You will provide one‑on‑one skilled nursing care in patients' homes and help implement care plans with physicians and clinicians.

This full‑time position offers pay per visit ($33–$47) and an annual range of $53,000–$73,000, plus Humana benefits. A valid nursing license, CPR, driving ability, and reliable transportation are required.

Qualifications

• Current nursing license in the practicing state.
• Valid driver's license, auto insurance, and reliable transportation.
• Current CPR certification.
• Two years experience as an LPN/LVN in a clinical setting.

Responsibilities

• Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN.
• Implement individualized care plans in collaboration with the patient, family, and healthcare team.
• Educate patients and their families on disease management, treatment options, and self-care techniques.
• Maintain accurate records of patient care, including documentation of patient visits, interventions, and outcomes.
• Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes. Participate in care coordination activities and discharge planning.

Position Overview

As a CenterWell Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you’ll manage care plans that support recovery and help patients get back to the life they love.

Required Qualifications

Compensation

Pay Per Visit/Unit Rate: $33.00 - $47.00

Pay Range: $53,000 - $73,000 per year (full‑time, 40 hours per week; may vary based on location and experience)

Additional Information

TB Statement
This role is patient‑facing and is part of Humana’s Tuberculosis (TB) screening program. If selected, you will be required to be screened for TB.

Driving Statement
This role is part of Humana’s driver safety program and requires a valid state driver’s license, personal vehicle liability insurance, and compliance with state minimum limits or higher.
