Qualifications

• High School Diploma or GED is required.
• Minimum 1 year experience in health care data entry.
• Reliable transportation, valid driver’s license, and automobile insurance required.
• Associates Degree preferred; home care or hospice information systems experience is a plus.

Responsibilities

• Manage patient intake and data entry to start the home health journey.
• Verify referral information and audit admission documentation for accuracy.
• Coordinate scheduling and communicate with patients, families, and partners.
• Monitor documentation to ensure timely admissions and regulatory compliance.
