Adoration Health is seeking an Home Health Intake Coordinator to guide patients from initial referral through admission, ensuring a smooth transition into home care. You will oversee patient education, plan of care initiation, and coordination with multiple service providers to deliver timely, quality care.

The role requires RN or LPN licensure, at least 2 years in healthcare, and strong knowledge of insurance verification and authorization processes.

Qualifications

• RN or LPN licensure in the state of operation
• 2+ years of healthcare experience preferred
• Strong written and verbal communication skills

Responsibilities

• Complete the intake process from referral to home care admission
• Oversee patient education and preparation for home care; initiate plan of care and coordinate with service providers
• Triag e referrals to determine timeline initiation of care based on clinical knowledge
• Coordinate insurance verification and authorization; communicate with patients and families
• Document demographics, medical condition, medications, and requested services
• Maintain relationships with referral sources and ensure compliance with state and federal regulations
