Arma Home Health Care Inc in Glendale, CA is seeking a dedicated Home Health Care Case Manager/Intake Coordinator to join our growing team. In this dual role, you’ll connect patients, families, and care teams to ensure seamless admissions and coordinated home health services.

You will manage the intake process, oversee care plans with clinicians, and serve as the primary contact for patients and insurers while documenting in the EMR/EHR system.

Qualifications

• Active RN or LVN license; case management or healthcare coordination experience required.
• Experience in home health, intake coordination, or case management strongly preferred.
• Knowledge of Medicare, Medi-Cal, and private insurance home health guidelines.
• Excellent communication and interpersonal skills with patients, families, and partners.
• Proficiency with EMR/EHR systems and standard office software.
• Bilingual English and Armenian is a plus in the Glendale community.

Responsibilities

• Manage the end-to-end intake process for new home health patients, including referral intake, eligibility verification, and authorization.
• Coordinate and oversee individualized patient care plans in collaboration with physicians, nurses, and therapists.
• Serve as the primary point of contact for patients, families, referral sources, and insurance providers.
• Monitor patient progress and ensure compliance with care plan goals and regulatory requirements.
• Facilitate smooth transitions of care from hospital, SNF, or clinic settings to home health services.
• Maintain accurate and timely documentation in the EMR/EHR system.
• Communicate with payers regarding authorizations, denials, and appeals as needed.
