Kona Medical Consulting and The Home Health Team in Texas is seeking a Home Health Field Registered Nurse (RN) to deliver skilled nursing care in patients' homes across various Texas areas.

The ideal candidate has an active Texas RN license, at least one year of nursing experience (home health preferred), and the ability to work independently with strong clinical judgment. EMR experience and reliable transportation are essential.

Qualifications

• RN license in Texas is required.
• Minimum 1 year of nursing experience; home health preferred.
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation with valid driver's license and auto insurance.
• Experience with EMR/mobile charting is preferred.

Responsibilities

• Conduct in-home assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Provide wound care, disease management, medication administration, patient education.
• Coordinate with physicians, case managers, and interdisciplinary teams.
• Complete accurate documentation in accordance with regulations.
• Participate in on-call rotation as needed.

Position Overview
