The ideal candidate has an active Texas RN license, at least one year of nursing experience (home health preferred), and the ability to work independently with strong clinical judgment. EMR experience and reliable transportation are essential.

Qualifications

• Minimum 1 year of nursing experience; home health preferred.
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation with valid driver's license and auto insurance.
• Experience with EMR/mobile charting is preferred.

Responsibilities

• Conduct in-home assessments and skilled nursing visits.

Position Overview

We are seeking a skilled and motivated Home Health Field Registered Nurse (RN) to provide high-quality skilled nursing care to patients in their homes. The ideal candidate is reliable, organized, confident in clinical decision-making, and passionate about helping patients achieve their health goals.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits
• Develop and update individualized care plans
• Perform wound care, disease management, medication administration, and patient education
• Communicate effectively with physicians, case managers, and interdisciplinary teams
• Complete accurate, timely documentation in accordance with regulatory requirements
• Participate in on-call rotation as needed
• Build trusting relationships with patients and families

Qualifications

• Current RN license in the state of Texas
• 1+ year of nursing experience (Home Health preferred but not required)
• Strong clinical assessment and critical-thinking skills
• Reliable transportation, valid driver’s license, and proof of auto insurance
