AHN Healthcare@Home is hiring a Certified Nursing Assistant for a Home Health position serving patients in the Bridgeville, PA area. You will provide compassionate care in patients' homes, assist with daily living activities, and support families as part of an interdisciplinary team.

Qualifications

• Active CNA certificate in the state you work.
• High school diploma or equivalent.
• Minimum one year healthcare experience.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.

Responsabilités

• Deliver compassionate, patient-centered care in patients' homes as part of an interdisciplinary team.
• Provide hands-on ADL assistance and observe/report condition changes for safety and independence.
• Educate and support patients and caregivers by reinforcing care plans and healthy routines.

We are hiring a Certified Nursing Assistant for a Home Health position providing care to patients in their homes in the Bridgeville, PA area.

What You’ll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence
• Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks
• Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR

• Work independently, but never alone
• Travel to patient homes within assigned territory
