Job summary

Residential Home Health and Hospice in Middletown, PA is seeking a Certified Nursing Assistant to provide compassionate hospice care in patients' homes. You will work with an interdisciplinary team to support comfort, dignity, and quality of life.

You will assist with activities of daily living, observe condition changes, document care in the EMR, and travel to patient homes within your territory. A valid CNA license and driver’s license are required.

Qualifications

• Active CNA certificate in the state you work
• High School Diploma or equivalent
• Minimum one year healthcare experience; hospice preferred
• Driver’s license and ability to drive ~20% of each workday

Responsibilities

• Deliver compassionate, patient-centered care in hospice, collaborating with an interdisciplinary team
• Provide hands-on care for ADLs and observe changes in condition
• Reinforce care plans, offer emotional support, and guide patients and caregivers

What You’ll Do:

• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory

CNA Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work

• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
