AHN Healthcare@Home is hiring a Certified Nursing Assistant for a Home Health position serving patients in the Bridgeville, PA area. You will provide compassionate, in-home care and support to patients and families, with autonomy and a strong interdisciplinary team backing you.

The role includes travel to patient homes within the assigned territory, including driving up to ~20% of the day. We offer education support, strong mentorship, and growth opportunities within a collaborative environment.

Qualifications

• Active CNA certificate in the state you work.
• High School Diploma or certificate.
• Minimum one year healthcare experience.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.

Responsabilités

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life.
• Provide hands-on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence.
• Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks.
• Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.
