Job summary

Residential Home Health and Hospice seeks a compassionate Certified Nursing Assistant to provide in-home hospice care in the Lancaster, PA area. You will support patients with ADLs, document care in the EMR, and collaborate with an interdisciplinary team to ensure comfort and quality of life.

The role offers autonomy, ongoing education, and opportunities for growth within a nationally recognized home health leader. Travel is required within the service area.

Qualifications

• Active CNA certification in the state you work
• Minimum one year healthcare experience
• Current driver’s license and the ability to drive to patient homes (~20% of day)

Responsibilities

• Deliver compassionate, patient-centered care in the home and hospice setting
• Assist with activities of daily living and observe/report changes in condition
• Educate patients and caregivers on care plans and daily routines
• Maintain accurate timely documentation in the EMR and communicate with the care team
• Work independently while collaborating with an interdisciplinary team
• Travel to patient homes within assigned territory

• Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR


• Work independently, but never alone
