AHN Healthcare@Home in Bridgeville, PA, is hiring a Certified Nursing Assistant to deliver compassionate in-home care within a hospice setting, partnering with an interdisciplinary team to support comfort, dignity, and quality of life.

You will assist with ADLs, observe and report changes, document in the EMR, and educate patients and caregivers about care plans. Travel to patient homes is required within the assigned territory, with a supportive culture and opportunities for growth.

Qualificações

• Active CNA certificate in the state of work.
• High school diploma or equivalent.
• Minimum one year healthcare experience.
• Valid driver's license and ability to drive ~20% of time.

What You’ll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence
• Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks
• Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR

CNA Qualifications
