Job summary

Residential Home Health and Residential Hospice in Lancaster, PA is seeking a Part-Time Home Health CNA to provide compassionate in-home care to patients, support ADLs, and collaborate with an interdisciplinary team to promote comfort and independence.

Travel to patient homes within assigned territory; complete timely EMR documentation and participate in ongoing education and growth opportunities with a supportive leadership team.

Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work.
• High School Diploma or certificate.
• Minimum one year healthcare experience.
• Current driver's license and ability to spend ~20% of your day driving to/from patient locations.

We are hiring a Certified Nursing Assistant for a Home Health position providing care to patients in their homes in the Lancaster, PA area.

What You'll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life

• Provide hands-on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence

• Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks

• Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR

• Work independently, but never alone

• Travel to patient homes within assigned territory
