AHN Healthcare@Home in Pittsburgh is hiring a Certified Nursing Assistant to provide in-home care in the East Liberty area. Join a nationally recognized home health and hospice leader offering autonomy, meaningful patient relationships, and strong career growth.

As a CNA, you will assist with ADLs, observe changes, document in EMR, and collaborate with the care team to ensure comfort and safety. A valid driver’s license is required for daily travel within the territory.

Qualifications

• Active CNA certificate in the state you work.
• High School diploma or certificate.
• Minimum one year healthcare experience.
• Valid driver’s license and ~20% travel to patient locations.

What You’ll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence

• Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory

CNA Qualifications

• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
